Visitor Education - Updated May 10, 2021
All visitors must review and complete the education on this page before they are permitted to visit. If you are one of the two Designated Visitors, it is your responsibility to ensure any visitor coming with you has also completed this education.
When you book and arrive for your safe visit, you will be asked to certify in writing you have completed this education and that you understand your responsibilities in preventing the spread of COVID-19 and enjoying a safe visit. If you have any questions or clarifications about this information, please speak with your site leader or email firstname.lastname@example.org.
The education consists of the following four activities:
1. First, familiarize yourself with the Shepherd’s Care Safe Visitation Policy, which outlines your rights and responsibilities as a visitor. The policy is available on out Safe Visitation Policy page at https://www.shepherdscare.org/visiting-loved-ones/site-visitation-policies/.
2. Review the COVID-19 Visitor Education Information presentation availablehere (updated May 10, 2021)
3. Watch the "Safe Access to Congregate Residents" video available on the AHS website here.
4. Check your understanding by practicing safe masking procedures and proper hand hygiene. Review the materials one more time and if you have any questions or clarifications about this information, please speak with your site leader or email email@example.com.
Once you have completed all four of these activities, you may self-certify that you have successfully completed the visitor education. Although you are not required to re-do this material each time you visit, it is a good idea to refer back to this page periodically to ensure your understanding remains current. If there are any material changes to this information, we will update this page.
