Shepherd’s Care Foundation received an Alberta Health Continuing Care Innovation Grant in 2012 to undertake the Campus of Wellness and Care Demonstration Project at Kensington Village.
There has been rapid growth of oldest seniors (85+), rising care costs and realizations that health and quality of life can be retained longer. These trends require a new focus on health promotion and prevention innovations that can change the path of declining health for seniors.
With the average age of 84 years old at our Kensington Campus, it was a perfect fit to test and demonstrate new ways of delivering services to provide choice for oldest seniors to age in place at a campus site. It also allows us to assist in provincial policy development.
We have implemented a number of programs or interventions as part of the program.
Independent Living residents have been given weekday access to a number of on-site services including:
These services have resulted in over 120 EMS calls avoided to date. Residents are also more aware and knowledgeable on various health issues and self-management of health conditions.
Independent and home care residents have helped plan and have been given access to on-site activities and supports. These physical, cognitive and social activities enable them to remain independent for as long as possible. Because of this program, an active living culture change is underway with our residents’ input, use of a health promotion and prevention model for functional aging and resident personal empowerment to maintain and improve their health.
Learnings include what supports and what prevents residents from participating or being active on their own. We also have fitness data that shows that seniors, regardless of age (even over 90), benefit from physical activity.
We are currently trialing different interventions to determine how to generate interest and motivate more residents to invest in their health and wellness by remaining active. We have found that relationship and encouragement are key, along with the use of strength-based and person-centred approaches including stages of change and motivational interviewing.
A third party evaluation company, Catalyst Research and Development Inc., is evaluating this project. The project will be completed on June 30, 2017.
