Maternity Leave Coverage--18 month term

Working out of both our Millwoods and Kensington Village Campuses, the Social Worker (SW) reports to the Manager, Best Practice and Education in the Care and Quality division   providing overall leadership, coordination and direction in  social work interventions and in the care of our Long Term Care Residents within the Mission, Vision and Values of SCF and standards of professional social work practice. The SW ensures compliance with continuing care health service standards, employment and safety-related statutes, legislation, codes and regulations and SCF policies. The SW provides support, advocacy, and choice to Residents and their families to ensure the highest quality of person centered care achievable within Shepherd’s Care Foundation’s Mission, Vision and Values, and Behavior Standards in conjunction with the standards of professional Social Work Practice. Additionally, the SW demonstrates ongoing competence utilizing knowledge, skill, judgment and evidence-based practice in caring for the older adult while committed to continuous quality improvement. 

 As the designated individual that  assumes the role of  Shepherd’s Care Foundation’s Information and Privacy Coordinator, role responsibilities will include handling with requests from Residents/Authorized Representatives who wish to examine, copy, or correct personal health information collected and maintained by SCF, while ensuring SCF compliance with the Health Information Act.


  • Assesses, plans, develops, implements, evaluates, communicates  and documents all social work interventions in accordance with the Resident’s plan of care to achieve  quality resident care;
  • Utilizes critical thinking, problem solving and decision making skills to provide competent, person centered care;
  • Participates in the development of plans, objectives and strategies for care services which are aligned with overall organizational strategic directions;
  • Participates in the development of care services and operational policies, procedures and programs in collaboration with appropriate team members, physicians, interdisciplinary team and other stakeholders;
  • Plans, develops, implements, and evaluates social work services in accordance with evidence-based practices;
  • Ensures social work practice complies with the organizational policies and procedures, continuing care health service standards, accommodation standards, relevant legislation and regulations, accreditation standards and regional health authority requirements; 
  • Ensures completion of all Resident legal documentation as it relates to Guardianship, Trusteeship, and Powers of Attorney as required;   Responds, investigates and reviews concerns and complaints from residents, families and staff in accordance with the concern resolution process;
  • Represents and participates in corporate, site and external committees and professional committees as appropriate;
  • Acts as the  Information Privacy Coordinator for Shepherd’s Care Foundation and complies with the relevant legislation;
  • Represents and participates in corporate, site and external committees and professional committees as appropriate;
  • Participates in performance measurement to achieve continuous quality improvement. 
  • Analyzes available data for  planning/monitoring purposes.
  • Ensures investigation and follow up in respect to any unusual incidents, near misses, complaints or reports of safety concerns; 


  • A Social Worker in good standing with the Alberta College of Social Workers; Baccalaureate Degree in Social Work preferred; 
  • Minimum of two (2) years experience in geriatrics and continuing care preferred 
  • Commissioner for Oaths (COA); Designated Capacity Assessor (DCA)preferred;
  • Demonstrated skills in leadership, communication, teamwork, concern resolution process and customer service
  • Demonstrated knowledge and commitment to person-centered care
  • Strong organizational, computer and presentation skills are required in this position.Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures is essential in this position.
