The LPN is responsible for the provision of personal care assistance, nursing procedures, medication administration and the supervision of Health Care Aides (HCA) and other non-regulated health care staff and implementing the resident care plan according for the group of residents assigned. Works with the resident/family and interdisciplinary team to meet the care requirements of residents and communicates concerns about specific residents or the group of residents back to the Site Supervisor and/or client care coordinator. The LPN provides support, advocacy, and choice to residents and their families to ensure the highest quality of resident focused care achievable within the Mission, Vision and Values of Shepherd’s Care Foundation and standards of professional nursing practice. Demonstrates ongoing competence utilizing knowledge, skill, judgment and evidence based practice in caring for the older adult. Ensures compliance with continuing care health service standards, employment and safety-related statutes, legislation, codes and regulations and SCF policies.  Provides support to the residents and families in an environment that promotes a state of wellness, independence and person-centered

  This position has the following key responsibilities:

  • Participates in the activities and resources of assigned care services to achieve quality resident care and while meeting the overall corporate plans and objectives;
  • Participates and provides the clinical care services in collaboration with the AHS Case Manager and in accordance with evidence-based practices;
  • Participates and responds as part of the resident care team  complying with the organizational policies and procedures, continuing care health service standards, accommodation standards, relevant legislation and regulations, accreditation standards and regional health authority requirements;
  • Responds, and participates in any investigations and/or brings forward any concerns and complaints from residents, families and staff in accordance with the concern resolution process;
  • Participates in any investigations and brings forward all unusual incidents, critical incidents  and medication discrepancies to ensure corrective action is implemented;
  • Promotes a safe work environment for residents, family members, co-workers and self.
  • Maintains an ongoing program of personal learning in order to maintain own competence. Seeks to benefit from opportunities for professional development and continuing education;
  • Actively supports, promotes and implements the mission, vision, values and strategic directions of Shepherd’s Care Foundation.


  • Licensed Practical Nurse, experience in dementia and gerontology required
  • Current membership in good standing with CLPNA
  • Requires three to five years leadership experience
  • Demonstrated skills in leadership, communication, teamwork, concern resolution process and customer service
  • Personable and compassionate in caring for the older adult
  • Demonstrated ongoing competence utilizing knowledge, skill, judgment, critical thinking and evidence based practice in caring for the older adult
  • Demonstrated knowledge and commitment to person-centered care
  • Commitment to team based collaboration and practice
  • Exhibits strong organizational, communication (written/verbal) and computer skills
  • Demonstrated commitment to the mission, vision and values of the Shepherd’s Care Foundation
  • Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures is essential in this position
